Attestation of Life Certificate (for retired pensioners) - All retired pensioners, who are required to submit ‘Life Certificate’ to their parent office, may apply for attestation of ‘Life Certificate’, duly filled-in.
- Attestation of ‘Life Certificate’ is done on GRATIS basis.
Documents required:- The following documents are required to be submitted for attestation of ‘Life Certificate’:- - Copy of Miscellaneous Service Application form, duly filled-in
- Life Certificate Form, duly filled-in, as prescribed by applicant’s parent office;
- Passports (first and last page) of the applicant and a photocopy thereof
- Copy of Visa/Stay permit and address proof
