Affidavit for a Child's Passport

Indian Nationals residing in Maldives should execute an Affidavit in order to apply for a passport for their new born child in India.

Procedure:

• Download application form from our website (Click here to download )  

• Please submit the form duly filled-up in duplicate along with following documents and photocopies thereof.

• Original Passport and Identity card (WP) of the applicant

• Copy of Birth certificate of the child

• Marriage certificate (copy)

• In case of renewal of passport, furnish a copy of the child's current passport

Fees : MVR 190/-

Processing time : One day

NOTE: The applicant has to come in person to the Embassy to execute the Affidavit
