Indian nationals living in consular jurisdiction of Consulate General of India, Phuentsholing and wishing to apply for passport may please personally submit their application form for fresh passport/ re-issue of passport (in lieu of expired) and duplicate passport (in lieu of lost/damaged/destroyed) along with following documents in original.
Following documents are required for:-
1. Issue of a fresh passport:-
(a) Age proof certificate (Birth Certificate or Matriculation Certificate)
(b) Proof of residence in India (Voter Id or Ration Card or Residence Certificate issued by District Magistrate)
(c) Educational qualification certificate
(d) Work Permit/ Dependent Card/ Trade Licence issued by Royal Government of Bhutan
(e) No Objection Certificate (NOC) from the head of organization/firm where the applicant is serving
(f) Employer's letter/ copies of electricity/ telephone/ water bills as proof of residence in Bhutan.
(g) Four passport size photographs (4cm x 4cm) to be enclosed
(h) Two sets of all documents- self attested.
Note- In case of minor, the applicant may please submit either of his parent's valid passport and a copy of passport along with above mentioned documents.
2. Re-issue of passport in lieu of expired passport-
(a) Original passport & a photocopy of first page and last page of passport.
(b) Work Permit/ Dependent Card/ Trade Licence issued by Royal Government of Bhutan.
(c) No Objection Certificate from the head of organization/firm where the applicant is serving
(d) Employer's letter/ copies of electricity/telephone/water bills as proof of residence in Bhutan.
(e) Four passport size photographs (4cm x 4cm) to be enclosed.
3. Issue of duplicate passport (in lieu of lost/damaged/destroyed passport)
(a) Copy of FIR lodged.
(b) Passport details such as No., Date of Issue, Place of Issue and validity,
(c) All documents required for fresh passport.
(d) A signed statement indicating circumstances in which passport was lost/damaged/destroyed.
