 Original and self-attested copy of valid Indian Passport (first and last pages)

Original and self-attested copy of valid Cyprus visa/ residence permit

Request Letter stating the purpose of Life Certificate

The issuance of the certificate is subject to verification of the Documents/credentials submitted by the applicant.

SUBMISSION OF APPLICATION FORMS:

Application along with supporting documents may be submitted at the High Commission personally.
