Attestation of Documents
(Time Required: Documents for attestation may be submitted at the Embassy of India, Tashkent between 0930 hrs and 1230 hrs on any working day and may be collected next working day 1400 hrs to 1500 hrs)

Fees will be collected separately for separate documents.
1) Sponsorship Declaration
Requirements for attestation of Sponsorship Declaration: -
- Misc Service Form (format 1)
- Photocopy of inside cover pages and other relevant pages of the passport.
- Copy of existing Visa/Residence Permit.
- Salary Certificate attested by Chamber of Commerce with typed English Translation.
- Original and one photocopy of the Sponsorship Declaration Form (Format

) to be signed before the Consular Officer of the Embassy.
2) Power of Attorney (Property Matters)
Requirements for attestation of Power of Attorney (Property Matters):
- Misc Service Form (format 1
) - Photocopy of inside cover pages and other relevant pages of the passport.
- Copy of existing Visa/Residence Permit.
- Two recent Passport Size (35 mm x 45 mm) photographs.
- Original and one photocopy of the Power of Attorney to be signed before the Consular Officer of the Embassy.
- Two witness of Indian nationality along with their original passport and copy of passport and their photos.
3) Power of Attorney (other than Property Matters)
Requirements for attestation of Power of Attorney (other than Property Matters): -
- Photocopy of inside cover pages and other relevant pages of the passport.
- Copy of existing Visa/Residence Permit.
- Original and one photocopy of the Power of Attorney to be signed before the Consular Officer of the Embassy.
- Two witness of Indian nationality along with their original passport and copy of passport and their photos.
4) Attestation of Medical Degree Issued from Uzbekistan
- Misc Service Form (format 1
) - Photocopy of inside cover pages and other relevant pages of the passport.
- Copy of existing Visa/Residence Permit.
- Confirmation letter from the Medical Institute or University for the following information:
- Period of study in recognized college with date of entry and date of leaving the institute.
- Period of study in unrecognized college, if any, with date of entry and date of leaving the institution.
- Date of transfer from one institution to another, if any.
- If there is more than one institution in which the candidate has studied, similar details of this fact may also be provided.
- Study Period of M.B.B.S. course from starting date to last date.
- The MBBS course was pursued partially or fully online or offline. If MBBSS Course was online then, has the individual successfully compensated all the practical studies which he/she might have missed during online period in physical form in the university and together he/she has given the final exams in person himself (i.e. offline mode).
5) Salary Certificate
Requirements for attestation of Salary Certificate: -
- Photocopy of inside cover pages and other relevant pages of the passport.
- Copy of existing Visa/Residence Permit.
- The original of the Salary Certificate duly attested by the Uzbek Foreign Office / Chamber of Commerce & Industry and a photocopy.
- One photograph to be pasted on the Salary Certificate.
6) Academic Degree / Certificate
Requirements for attestation of Academic Degree / Certificate: -
- Original Passport (including subsequent additional booklets if any)
- Photocopy of inside cover pages and other relevant pages of the passport.
- Copy of existing Visa/Residence Permit.
- If original degree / certificate is to be attested:
(i) Original of the Degree/Certificate duly attested by the General Administration Department of the concerned State/ Ministry of HRD and the Consular Section, Ministry of External Affairs, Patiala House, New Delhi and a photocopy.
(ii) Original of the Mark Sheet and a photocopy. - If a copy of the degree / certificate is to be attested:
(i) Original of the Degree/Certificate duly attested by the General Administration Department of the concerned State and the Consular Section, Ministry of External Affairs, Patiala House, New Delhi and two photocopies.
(ii) Original of the Mark Sheet and a photocopy.
Requirements for attestation of Transfer Certificates originating in Uzbekistan: -
- Original TC and a copy of the same. The Original TC is required to be attested by Ministry of Foreign Affairs / Ministry of Education.
- Passport copy of the child.
- Copy of Residence Permit (if available).
7) Marriage Certificate
Requirements for attestation of Marriage Certificate
- Photocopy of inside cover pages and other relevant pages of the passport.
- Copy of existing Visa/Residence Permit.
- The original and a photocopy of the Marriage Certificate duly attested by the concerned State Government / Ministry of External Affairs, New Delhi.
8)Bank Account Opening Form
Requirements for attestation of the NRE Account Opening Form: -
- Photocopy of inside cover pages and other relevant pages of the passport.
- Copy of existing Visa/Residence Permit.
- Original and one photocopy of the Account Opening Form as prescribed by the concerned Bank/Institutionto be signed before the Consular Officer of the Embassy.
9) NRI Certificate:
Note : NRI Definition: An individual is an NRI if they've been in India for less than 182 days in the preceding financial year or if they've been in India for less than 60 days during the previous year and 365 days or less during the past four years.
Requirements for attestation of the NRE Account Opening Form:-
- Photocopy of inside cover pages and other relevant pages of the passport.
- Copy of existing Visa/Residence Permit.

10) Life Certificate
Fee: Gratis
Requirements for attestation of the Life Certificate (for pensioners):-
- Photocopy of inside cover pages and other relevant pages of the passport.
- Copy of existing Visa/Residence Permit.
- Original and one photocopy of the Life Certificate in the Format prescribed by the concerned Bank/Institution to be signed before the Consular Officer of the Embassy.
